Samsara provides the Hootfund program designed to support and encourage a well-rounded healthy lifestyle for all Samsara employees. The program covers expenses for a variety of eligible wellness activities. This document outlines the program policy.
All full-time employees are eligible to participate in the program. You must be employed by Samsara when eligible expenses are incurred (paid for).
Forma is the administrator for the Hootfund program. For general FAQs, please refer to the help center. You can reach Forma’s Member Experience Team by emailing [email protected] or through the live chat feature directly in your Forma account
All Samsarians are eligible to receive the Hootfund for eligible expenses for up to $150 (or equivalent to local currency) per quarter. Any unused amount expires at the end of each quarter. Please refer to the run-out calendar below.
The Benefit Amount (see table below) will be deposited into your Forma Account on the 1st of each fiscal quarter.
You may spend the allowance through any of the two following ways:
Forma Store includes vendors that are automatically considered eligible under this program policy. You do not need to submit a request for reimbursement when you purchase products through Forma Store.
For a single-time purchase that exceeds your Forma balance at any given time, you may pay the difference out of pocket via the Forma portal and submit a receipt for reimbursement as described below.
